How do I sign up for Ballad Health?
Once you log in through your computer or the Ballad Health app, look for Sign Up Now on the login page. Choose the Activation Code option. Enter your code, the last four digits of your Social Security number and your date of birth. Click Next to continue with the enrollment process and create your username and password.

Why do patients need to be transferred to another hospital?
Sometimes patients need to be transferred to another hospital to receive a higher level of care or specialty services. If the doctors decide a patient transfer is in the patient’s best interest, we will work with you to facilitate as smooth a transfer as possible.

What is a psychotherapy note?
Psychotherapy notes, which are the personal notes of a mental health care provider documenting or analyzing the contents of a counseling session, that are maintained separate from the rest of the patient’s medical record. Information compiled in reasonable anticipation of, or for use in, a civil, criminal, or administrative action or proceeding.

Can a covered entity send phi to another person?
An individual also has a right to direct the covered entity to transmit the PHI about the individual directly to another person or entity designated by the individual. The individual’s request to direct the PHI to another person must be in writing, signed by the individual, and clearly identify the designated person and where to send the PHI. The same requirements for providing the PHI to the individual, such as the fee limitations and requirements for providing the PHI in the form and format and manner requested by the individual, apply when an individual directs that the PHI be sent to another person.

How to access a child's medical record?
Parents or legal guardians may request access to their child’s medical record by completing a proxy access form at their provider’s office. For patient's between 12- and 17-years old, there will be limited access due to regulations set by the states of Tennessee and Virginia.
What is MyChart proxy?
MyChart makes it easy to manage your family’s care in one place. Access can be granted for parents who want to view their children’s records or for adult patients wishing to grant access to other adults. As a proxy, you can take advantage of MyChart features as if you were that person.
What is an e-visit?
An E-Visit is a convenient way to contact with your provider for common, non-urgent medical conditions. You can submit a questionnaire regarding your symptoms for a variety of conditions and receive a written response through your MyChart account. A Ballad Health provider will respond during office hours.

Is there a fee for using MyChart?
There is no fee for using MyChart.
Is Ballad Health secure?
Yes, Ballad Health takes great care to ensure your health information is kept private and secure, including E-Visits and your MyChart activity. Your account can’t be accessed without your unique password. Also, the MyChart system uses the latest 128-bit SSL technology to automatically encrypt your E-Visit session.

For example, if you visited the Franklin Woods Community Hospital emergency room today, the ER doctor could see what medications your Ballad Health Medical Associates family physician prescribed to you at your last annual visit.

Remember: You must be a current or past patient and have a medical record at a Ballad Health hospital or clinic to register for MyChart.
Does Wellmont have a MyChart?
If, in the past, you set up a MyWellmont account with a legacy Wellmont Health System facility, your health records have transferred over to what is now called MyChart. You will just need to update the MyWellmont app through your app store.
