To request access to Baptist Gateway, simply complete the BAPTIST GATEWAY PATIENT PORTAL ACCESS REQUEST FORM. This request form must be sent to [email protected] from a valid email address (cloud messages are not accepted) or returned in-person to the facility's Health Information Department from which you are requesting your records. Please allow 7 to 10 days to process your request.
