What is the Calderdale Royal Hospital?

Calderdale Royal Hospital (CRH) is a modern centre of excellence which first opened its doors to patients in April 2001. Situated about 1.5 miles south of Halifax town centre the hospital offers a full range of day case and outpatient services and an accident and emergency department. It is also the specialist centre for planned orthopaedic and general surgery (when patients need to stay at least one night) for the residents of Calderdale and Kirklees and further afield.

How to request access to health records?

All such requests must be made via the Access to Health Records Department. The request should contain adequate supporting information such as your full name, address, date of birth, NHS number, etc. This is required so we can verify your identity and locate your records. If you wish to do this via a form, you can get the form by clicking here

When can you register for an outpatient appointment?

1. You can register when you attend any Outpatient appointment or if you are admitted to our hospitals as an inpatient.

What is appointment service?

Appointment Services: Customer Care, negotiation / booking of New and follow up outpatient appointments.

How to get a copy of a clinic letter?

The most efficient way to view a copy of your clinic letter is by registering for the YourEPR patient portal. The only other way to receive a copy would be to place a request through Access to Data. They can be contacted on 01422 222065 or by email [email protected]

Why do we have an appointment reminder?

A: As well as patient appointment communications, we run an appointment reminder service so patients remember the key details of their upcoming appointment. Once the appointment has been confirmed on a smartphone, it will automatically link to the mobile phone calendar.

What is reception in medical?

Reception: Reception duties for all outpatient clinics, booking of follow up appointments, customer care and admissions of elective patients.

What happens if a digital letter is not accessed?

We know not everyone will choose this option, so if the digital letter is not accessed within 24 hours, or no mobile number is held, a letter will be automatically sent in the post. The digital letter will also be deleted.

What do you do if you are an authorized caregiver?

If you are an authorized caregiver that needs help accessing a family member’s account, you will need to contact that family member’s medical provider for support.

Can you use a portal locator on a patient portal?

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.
