The My Chesapeake Urology Patient Portal is supported by our partner vendor, Follow My Health. Office Policy and New Patient Forms
To help you get acquainted with our office and our policies, please read our Office Policy and New Patient Package Forms below. Please take a few moments prior to your appointment to review and complete these registration forms. We ask that you bring the completed forms with you to your appointment, along with your insurance card.
Appointments
We request that appointments be made in advance. Please make sure you bring your photo ID, insurance card, and a bag with all of your prescription medications, herbal supplements, and over-the-counter medications. Please arrive at least 15 minutes before your scheduled appointment time.
No Show
If you fail to show three times for your scheduled appointment, you may lose the privilege of scheduling your appointment in the future, and will have to walk in to the office and wait to be seen by a provider.
Office Hours

Parents/legal guardians must register for a MyCHKD account in person at the pediatrician’s office. This only takes a few minutes. After that, you will receive an e-mail at the address you provide with instructions on how to set up an account. Please note: if you provide a shared e-mail address, anyone you share that address with will also be able to see the e-mail.
