Do med/peds doctors see the same patients as family practice doctors?
Med/Peds doctors see the same types of patients as Family Practice physicians. However, Med/Peds doctors receive more intensive training in Pediatrics and Internal Medicine, enabling them to provide specialized care for the whole family.
What is an ABN at Creekside Medical?
If you choose to receive services that will not be covered by your insurance plan, (or are not likely to be covered), you may be asked to sign an Advanced Beneficiary Notice (ABN) or a Notice of Patient Financial Responsibility form as an agreement that you are responsible for payment.
What information is required to request medical records?
A person requesting medical records must submit a written consent with the following information: Patient name, date of birth, contact information and last four digits of your SSN. Information being requested and dates of service. The name and address of the person the information is being released to.
What to do if you have any questions about your medication?
If you have any questions regarding your medications please discuss these during your appointment. If for any reason you feel your medication needs to be adjusted or changed please contact your provider immediately
When do you have to notify your doctor of refills?
Medication refills will only be addressed during regular office hours. Please notify your provider on the next business day if you find yourself out of medication after hours.
Can you release medical records without a signed authorization?
PLEASE NOTE: Release of information or medical records requests must comply with state and federal guidelines. Records will not be released without a signed authorization from the patient or the patient’s legal representative.
Does Creekside Medical pay in cash?
Creekside’s payment policy states that payment is due upon receipt of your first statement. Unless covered by insurance, payment can be made in cash, by personal check, or by credit card. There is a charge for checks returned for non-sufficient funds.

You are welcome to call the clinic with medical questions about yourself or another clinic patient for which you are a parent or guardian. We make every effort to return phone calls by the end of the business day. Please schedule an appointment for medication requests, medication changes, and for any illness or symptom that may require treatment.

All our physicians have extensive training and experience in diagnosing and treating diseases in every age group, from infants to the elderly. Lon Haskell, MD; Michael Honeywell, MD; Ross Summerford, MD; Christopher O’Halloran, DO; and David Showers, MD treat a wide range of conditions, such as diabetes, cardiovascular disease, and osteoporosis and provide wellness preventative services.
