Teaching Hospital

How to request a copy of medical records from East Liverpool Health System?
You may also write a letter authorizing East Liverpool Health System to release a copy of your records. In the letter, you must state the patient's name, date of birth, social security number and date (s) of service. You must sign and date the letter and specify the information to be disclosed. The letter should also state the name (s) of the person (s) to whom the information may be released by the HIM Department. The letter must be signed by the patient on whom the medical record is maintained, or by a person lawfully authorized to act on the patient's behalf. The letter of request must be dated.

Formerly known as Medical Records, the Health Information Management Department (HIM) maintains patient medical records.
Can a patient release his/her medical records?
Only the patient can release his/her records. However, if you have the power of attorney or are the executor of a patient's estate, you may submit a request in writing along with a copy of the legal document stating your role.

Your primary care provider can perform many basic procedures that will help you avoid the cost of seeing a specialist . An example of some of the procedures RVP providers perform includes tests for throat cultures, stiches for minor wounds, injections to treat pain and inflammation, mole biopsies and wart removal.
How to sign up for patient portal?
When you appear at a clinic or have a hospital visit, you will be asked if you would like to sign up for the patient portal. Provided you give us a photo ID and a valid email address, you will receive an email invitation with a link that connects you to another secure site where you set up your portal user account. Once you’ve followed the link you will be given directions on how to complete the registration process.

What if I forget to follow the email link?
You must follow the link to complete the registration process. If you do not complete the registration process within 90 days, the link allowing you to register will expire. You will then need to appear in person again to restart the process.
