Blount Memorial Hospital now offers an Online Patient Record Request tool for individuals to request patient records for themselves, their minor children or for caregivers acting on behalf of a patient (i.e. power of attorney).

Hospital records from before July 2018 can be obtained by calling Covenant Health Information Management at (865) 374-5269.
