Step Two - Complete Your Student Health Insurance Requirement

All new, transfer and continuing degree-seeking and all international Emory University students (including Oxford College) are required to have health insurance.

Step Three - Privacy Practices

Your Privacy form will be completed via the Student Patient Portal at https://www.shspnc.emory.edu. You will sign in with your standard Emory (OPUS) user ID and password (please be aware that your Emory password should be kept confidential. If you share it, others will potentially have access to your personal health information).

How to request a declination?

To request a declination, go to the Student Portal and find ‘Forms’ on the left side, then ‘Immunization Forms”. Scroll down to the bottom of the page and click on the link to the declination form. Upload the completed form. For a medical contraindication we need a letter from your provider explaining the contraindication. The letter needs to be signed by your provider and yourself. Upload the form through the student portal.

Can you submit a letter of explanation to Emory?

If you or your healthcare provider feels that you cannot comply with any of Emory’s immunization requirements (including medical contraindications to specific vaccinations), please submit a letter of explanation signed by both you and your healthcare provider along with your updated immunization records using the Student Patient Portal to upload.
