Fairfield Medical Center and Community Heart Watch – a committee dedicated to raising awareness and improving survival rates of sudden cardiac arrest (SCA) – recently received a $9,950 grant from the Fairfield County Foundation that will be used for education and the purchase of automated external defibrillators (AEDs) for the community.

How do I sign up for the medical records portal?

The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.

Where to fax medical records?

If you wish to personally pick-up the copies, please bring a photo ID (driver’s license, state ID, employment badge, etc.) to the Medical Records Department, located on the first floor at Fairfield Medical Center.

What is the phone number for release of information?

If you are attempting to reach a Release of Information Technician between the hours of 7 a.m. – 6 p.m. on Monday through Friday, please call 740-687-8264 or 740-687-8265.

Can you fax a medical record to a physician?

Include the phone number and fax number, if you wish the information to be sent to a physician. We typically only fax records to physician’s offices, if they are needed sooner than can be sent by mail.

Our Patient Portal allows you to make requests (for medical records, prescription refills, and specialist referrals), correspond with your doctor (send them a message and book an appointment), view your lab results, update your personal information, and view a summary of your visits..

Signing Up

Your doctor's office will provide you with a username and password to give you Patient Portal access. You can also get access by calling our Customer Service Department at (614) 423-6916 or toll-free at (844) 245-5538. The first time you log in, you will need to review the consent forms and click on next, then check the box, and click on agree.

Telehealth Visits

In an effort to prevent the spread of disease, we are working to offer virtual Telehealth visits instead of in-person visits for certain situations. You can participate in virtual Telehealth appointments if you have portal access and have a smart phone or computer with a camera. Learn About Telehealth Visits
