TRICARE Online Patient Portal Secure Messaging allows you to directly contact your team through the web. The Patient Centered Medical Home (PCMH) is a team-based model built around the premise that the best health care begins with a strong primary care foundation.
What is SCMH in the Army?
SCMH is the Soldier's version of a Patient Centered Medical Home (PCMH) which provides 90% of all Soldier care on site and is the same as all other PCMHs except SCMH's have integrated medical staff from line units and Military Treatment Facilities (MTFs) to make up a full team. It also has augmented behavioral health and physical therapy. A SCMH is the Brigade equivalent to "PCM Home." All SCMHs must achieve level two or three Army PCMH recognition by the independent National Committee for Quality Assurance (NCQA).

Your provider may offer education on wellness and health promotion and remind you of visits and age-appropriate wellness screenings.
What are the members of PCMH?
What members are included in my PCMH medical team? PCMH team members include a Primary Care Manager (PCM) — a physician, physician assistant or nurse practitioner — as well as a nurse, medic, nursing assistant, medical assistant, medical support assistant (clerk), pharmacist, social worker, case manager, and nutritionist.
Is there a way to receive care?
There are multiple ways to receive care in the way that works best for you. We have learned from our experience and listening to you, that both face-to-face and non-face-to-face encounters (virtual care) are needed. Although some medical needs require an appointment, many things can be more conveniently coordinated using technology.
Can you see your PCM at each visit?
The team will make sure you are able to see your PCM or a member of the team at each visit.

Good medical care means more than merely treating episodes of illness. Periodic health survey for the protection and early detection of illness is equally important. This consists of a review of one's routine tests and consultations with a physician. All patients should be seen annually for preventive care.

Do you need a referral for a follow up radiology test?
Please contact our office if you receive notification from the radiology facility that additional or follow-up testing is recommended. You will need a new referral or prescription for additional studies, and may need an appointment to discuss further.
