What is tricare portal?

TRICARE Online Patient Portal Secure Messaging allows you to directly contact your team through the web. Through this service, patients can initiate a web visit consultation, receive preventive care reminders, send a note to the provider’s office, request test results, and ask for prescription renewals. The Patient Centered Medical Home (PCMH) is a team-based model built around the premise that the best health care begins with a strong primary care foundation.

What is SCMH in the Army?

SCMH is the Soldier's version of a Patient Centered Medical Home (PCMH) which provides 90% of all Soldier care on site and is the same as all other PCMHs except SCMH's have integrated medical staff from line units and Military Treatment Facilities (MTFs) to make up a full team. It also has augmented behavioral health and physical therapy. A SCMH is the Brigade equivalent to "PCM Home." All SCMHs must achieve level two or three Army PCMH recognition by the independent National Committee for Quality Assurance (NCQA).

Do you need a referral for a follow up radiology test?

Please contact our office if you receive notification from the radiology facility that additional or follow-up testing is recommended. You will need a new referral or prescription for additional studies, and may need an appointment to discuss further.
