To enroll in mycareDOT™, you must first be an established patient of Clifton Park Family Medicine. You will need to provide our office with your name, email address, date of birth, and address. We will then send you an email inviting you to join the portal.

See one of our staff members and simply give us your email and we can sign you up. You will receive an email with directions on how to set up your portal account. Once you have completed the setup, you will be ready to take advantage of the portal immediately.
