You can sign into MyChart, our online patient portal. You can use the portal to view your records, request medication refills, access test results, manage your appointments and more.
Can I visit my loved one in the hospital during the pandemic?
We update our hospital visitation every Wednesday, based upon the risk of COVID-19 spread, the number of COVID-19 patients, hospital staffing and equipment levels.

Why use this billing process?
Patients admitted to the hospital or seen in our outpatient departments have typically received separate billings for hospital staff and physician services. Following this same type of billing process ensures more appropriate payment for the care provided.
Does this mean patients will pay more for services?
It’s possible some patients will pay more for certain outpatient services and procedures at our hospital outpatient locations, depending on the services they receive and their specific insurance coverage.
Does this affect patient co-pays or deductibles?
Depending on each patient’s specific insurance benefits, additional out-of-pocket expenses may be incurred under the “Hospital-Based Outpatient” model.
What is an adult proxy?
Adult-Adult proxy access allows another adult full access to the HMH MyChart account of a Hackensack Meridian Health patient who is 18 years of age or older. In order to obtain access, both the patient and proxy must complete and sign the HMH MyChart Adult Proxy Form. The proxy's access is terminated when the patient makes a written request to terminate access or revokes access from within their MyChart account.
Can a child have access to MyChart?
Access to the MyChart of account of children 11 and under is automatically available to parents and legal guardians. The child's physician office can grant that access. Access to MyChart accounts of children 12-17 can be granted by filling out the HMH MyChart Teen Consent Caregiver/Proxy Form. The teen, parent and provider must review, agree and sign the consent before access is granted. Proxy access is automatically terminated on the teen's 18th birthday. The teen will still have access but the guardian (proxy) will no longer have access.
