What happened to southern New Hampshire Health’s Ecare portal?

Southern New Hampshire Health has transitioned from offering the eCare patient portals to SolutionHealth MyChart. This transition is part of a substantial upgrade to our medical records system to Epic. SolutionHealth MyChart consolidates your medical history into one record available across Southern New Hampshire Health and Elliot Health System.

How do I Check my southern New Hampshire health appointment notes?

If you received care at Southern New Hampshire Health, those appointments, notes, and lab results were seamlessly and securely transferred and will now appear in your SolutionHealth MyChart portal. To check out SolutionHealth MyChart, visit mychart.solutionhealth.org and log in using your existing username and password.

Do I need to provide identifiable health information to access myhealthrecord?

You are not required to provide identifiable health information to visit the MyHealthRecord.com website. However, if you are a patient or the legal representative of a patient, you must provide certain identifiable health information in order to complete your registration for the Service and to access the health records maintained by your Provider.

Can I get access to my own health records?

Access to your own health records is provided for under the Data Protection Act 2018 and the General Data Protection Regulations 2018. Applications to access health records of patients who have died are made under the Access to Health Records Act 1990.

A Personal Health Record (PHR) is a summary document of your Core Physicians’ health history and can include items such as; allergies, medications, conditions, lab results, vital signs and more. To open and view your PHR as a PDF, you must have Adobe Acrobat Reader installed on your computer. You can also download and save your PHR.

If you are a new or established patient of Core Physicians, you can choose one of three ways to enroll in the Patient Portal: 1. Visit any Core Physicians’ office. 2. Call our Customer Service line at 603-580-6753. (Weekdays, 7:30am - 5:00pm) 3. Complete an Online Enrollment Form.

Can you use the patient portal if you have opted out of NHIIS?

If you previously opted out of NHIIS, you will not be able to use the patient portal because your information was not recorded in NHIIS. If you would like to change your opt-out status to access your immunization information through the portal, please contact your primary provider to request to change your opt-out status. If you don’t have a primary care provider, call (603) 271-0301 for assistance.

Is Elliot Health System a solution health system?

For many years, Elliot Health System patients have had secure online access to their electronic medical records through the MyEChart patient portal. Moving forward, MyEChart will be called SolutionHealth MyChart and will be available at both Elliot Health System and Southern New Hampshire Health, founding members of SolutionHealth.
