A patient portal account is an online tool that gives you anytime access to your personal health records and enables you to take proactive role in managing your care. FollowMyHealth® by Allscripts is Cardiology Associates’ patient portal provider.
How do I contact my doctor's office or follow my health?
For specific questions about making payments or the health information in your online record, please call your doctor's office. For the MyChart portal, call 864.512.3742 or send an email to [email protected]. For the FollowMyHealth portal, call 888.670.9775 or send an email to [email protected].
How do I sign up for a FollowMyHealth patient portal?
If you would like to access your personal Cardiology Associates health records online, please use the sign up button below or call our office at 251-607-9797 to request an invitation to sign up for a FollowMyHealth patient portal account. Scheduling appointments
Office visits are by appointment only. Appointments may be scheduled by contacting the clinic at (931) 433-2551. Be prepared to provide:
Canceling appointments
To cancel an appointment, please notify the clinic at least 24 hours in advance. This is an important courtesy to others waiting to schedule an appointment.
Immunizations and injections
Visits for immunizations or injections only do not require a regular appointment with the physician. However, please call us at least one day in advance so we can make sure to have adequate supplies and staff to assist you.
Prescription refills
Refills should be requested during regular office visits. Please allow 72 hours for refills to be completed. Your options for refills outside of office visits include:

Most insurance policies are accepted including Medicare, Medicaid, Tricare and Blue Cross/Blue Shield.
