CANCELLATION POLICY

If for any reason you are unable to keep your appointment please notify us 24 hours prior to your scheduled visit. If you are unable to cancel or reschedule your appointment 24 hours prior to your visit you will be charged a $50 cancellation fee. Please call our office for immediate assistance.

FINANCIAL NOTICE

Co-pays and deductibles are due upon arrival for your appointment. There is a $40 charge for all returned checks, including non-sufficient funds. There will be a $15 charge for each form brought in for completion. Payment is due when the form is presented. If you are late for your appointment, we may have to reschedule your appointment.

INSURANCE

Insurance is accepted at Internal Medicine Physicians. Please bring your current insurance card (s) to your visit so we can verify billing information, along with a picture ID. Co-pays are due at the time of your visit.
