How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

Does Jacinto Medical Group accept HMO patients?

Patients on an HMO plan must select a Jacinto Medical Group primary care physician with their health insurance provider in order to access all of the services available at Jacinto Medical Group. Primary care physicians are available throughout JMG locations in Baytown and La Porte. Please note that some physicians may not be accepting new patients at all times.

Siraj is a Chartered Accountant with a Bachelor of Commerce degree.

Chintan Shukla

Chintan has been with JMG since 2002 and held several roles in the clinic starting from Programmer, Systems Administrator, IT manager to managing several departments at Baytown facility. He currently oversees IT, Specialty, Imaging, Sleep, Purchasing, Maintenance and Clinical Quality Excellence department.

Irene Garcia

Irene has been with JMG since 1996 serving as a Referral Specialist and Acting Manager for family practice and Manager for Jacinto MRI and Diagnostics Center. She is married with two children and four grand children. She volunteers for church, loves to go dancing, and talking to friends on Facebook.

Our family physicians provide routine health checkups and counseling. They help you to make healthy life style choices, maintain or improve health, and prevent illness. If you have issues that require special attention, our family physician can recommend a doctor who has the expertise you need.

How to grant access to MyHealthHome?

To grant a caregiver, guardian or trusted family member/friend access to your MyHealthHome account, the requestor will first need to create their own MyHealthHome account. Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.
