Please Note: Online access to patient medical information for visits that occur on or after September 28, 2019 will be accessible through our new patient portal MyJeffersonHealth. Historical medical information will still be accessible from either the hospital portal or physician portals, respectively.

What is an EHR?
Your electronic health record (EHR) is an electronic version of your health information that is maintained by your doctors. Your health record may include a collection of your past doctor visits, treatments received, test results and medications. Accurate, real-time health records are a vital part of receiving the best care possible. Right now, your health record information may be spread across multiple doctors and hospitals that you have visited in the past. For your health care providers to make the most effective decisions on your care, your providers need to be able to see information about you such as your conditions, test results, treatments and the medications you may be taking.
