If you are requesting medical records from Women & Children’s Hospital of Buffalo or Oishei Children’s Hospital, please download the Release of Information Form ( in English | in Spanish ), print a copy, fill it out, and send to the appropriate contact address below.

Even though the hospitals of Kaleida collect information, complete the original birth certificates, and submit the information to the New York State Department of Health, the birth certificates are maintained by the Department of Vital Statistics in your city or town. All requests for copies of birth certificates must be made to these departments.
