Students can find downloadable forms, upload forms into their medical record and print immunizations records from the Patient Portal. Students use their Lafayette credentials to log in. For serious or life-threatening problems, call x4444 and request an ambulance. Sexual assault counselors also are available.

Is Bailey Health Center partnering with CVS Pharmacy?
Bailey Health Center has partnered with CVS Pharmacy to assist us in educating and vaccinating the students on campus. The event is still in the planning stages. Students should look for an announcement in the Lafayette Today. Employee Flu Clinics will be provided by Bailey Health Center Staff on September 28th & 29th.
