How do I designate a proxy to view my medical records?
Adult patients can designate a proxy to view their medical records. A proxy form must be completed and signed by both parties. A legal guardian of an incapacitated adult patient may also be granted proxy status. Proper copies of guardianship must be on file within the patient’s medical record. Please review all rules regarding proxy.
How do I make sure I am receiving patient portal emails?
To ensure that you are receiving Patient Portal emails please do the following: Add NextMD.com to your contact list, address list, safe list, or "Do Not Block" list. Account will be locked for 20 minutes after 4 failed login attempts. Please remember that password is case sensitive.
