Looking for Immunizations?

Immunizations are no longer available in Phoenix Children’s patient portal at this time. To access immunization information visit the Arizona Department of Health Services (ADHS) webpage. Please note this may not be a complete immunization record due to limitations with reporting to ADHS.

This can also be terminated at any time by calling the technical support center. In the future we hope to add additional functionality that allows you to add additional information related to your health to your My Portal (Patient Portal) account. If you do so, you'll have your information in a secure place that can be accessed anywhere you have Internet access.

Technical assistance is available 24 hours a day, seven days a week, (844) 274-8497. Technical support staff can help locate the invitation, provide assistance enrolling, logging in and navigating within My Portal (Patient Portal); however they are unable to assist with clinical questions.

What to do if you don't receive an invitation to my portal?

If you do not receive an invitation please contact the Medical Records Department. This web based tool may be in addition to online patient access offered by your doctor’s office.

Can I create an account for a family member who has a Dignity Health doctor?

Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a My Portal (Patient Portal) account with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age. For a dependent adult, they will need to sign a release, or you will need to provide a copy of the Medical Durable Power of Attorney before an account can be created for the dependent adult.

Does HonorHealth have an app?

If you'd like to use an app to access and import your health information from MyChart, HonorHealth gives you the ability to exchange information. You must have an active HonorHealth patient portal (MyChart) account because the security information you set – your username and password – will be your authentication for MyChart to share designated information with the app. Another security feature is a time limit for the session connection between the app and your MyChart.

How much is a no show fee for a psychiatric appointment?

If the Patient fails to cancel their appointment at least twenty-four (24) hours in advance, a $50 no show fee will be applied. This fee must be paid prior to being seen for any Medical, Dietetic, Psychiatric and/or Counseling appointment.
