Who are the primary care associates?
Primary Care Associates (PCA) is comprised of six physicians. Drs. Brad Wainer , Nejat Destani , Michael Gershberg, Matthew Hsieh and Dr. Nadia Momin specialize in Family Medicine. Dr. Lori Weiss specializes in Pediatrics. All of the Physicians are Board Certified in their specialties and are on staff at both MacNeal Hospital and LaGrange Hospital.

How do I request additional family members to access the portal?
Please log in at the Portal Login. Once logged in, on the left side of your home page under Messages, click on the hyperlink [CLICK HERE] to request access for additional family members. Submit your request and it will be processed within 48 hours by our staff.
