If you need a PIN, you can request one in-person at the Medical Records department at MelroseWakefield Hospital (Monday-Friday from 8 a.m.-4 p.m.) or by completing and faxing the Authorization to Use & Disclose Protected Health Information form with a copy of a photo ID to 781-979-3217.
