How to access my health record?

To access your record online (or someone else's where you have proxy rights) you need a patient portal account that is linked to MyHealth. If you do not have access, please sign up by creating a MyHealth account.

Where to bring form for hospital visits?

For access to hospital visits, please bring form to the hospital’s Registration department to be forwarded to the appropriate personnel

Notification: You have the right to have a family member representative and your physician notified of your admission to the hospital.

How to protect your privacy on FollowMyHealth?

To help protect your own privacy don’t access the portal on a work computer because your employer could be able to view your records. Unless it is absolutely necessary do not access the portal on a public computer. If you must use a public computer to access the FollowMyHealth portal, make sure you erase the computer’s browsing history. Always make sure you close the portal when you are finished so others can’t see your information. Don’t tell anyone your password, and if you do think someone had discovered your password change it immediately. Use the portal to send messages because email may not be secure.

How to appeal a hospital discharge?

Right to Appeal Premature Discharge (For Medicare Beneficiaries): Your Right to an Immediate Appeal without Financial Risk: When you are advised of your planned date of discharge, if you think you are being asked to leave the hospital too soon, you have the right to appeal to your Quality Improvement Organization ( also known as QIO). The QIO is authorized by Medicare to provide a second opinion about your readiness to leave. You may call Medicare toll-free 24 hours a day at 1-800-633-4227) or TTY/TTD: 1-877-486- 2048, for more information. If you appeal to the QIO by noon of the day after your receive non-coverage notice, you are not responsible for paying for the days you stay in the hospital during the QIO review , even if the QIO disagrees with you. The QIO will decide within one day after it receives the necessary information. You may need to speak with a Care Coordinator for assistance.
