Make an Appointment

Patients are seen by appointment. We will do our best to accommodate same day appointments whenever possible. To schedule an appointment, please call our office during normal business hours or utilize our online scheduling tool.

New Patients

We ask new patients to arrive 30 minutes prior to complete registration. You can download and print your new patient paperwork to complete and bring with you to your appointment. Please bring a photo ID and your insurance card with you to your appointment.

Cancellations and Rescheduling

We kindly request a 24 hour notice to cancel or reschedule appointments. This allows us adequate time to accommodate other patients who may be requesting to be seen

Billing and Payment

Payment is required at the time of service. Our office will file claims to most major insurance companies, however it is the patient responsibility to know and understand their benefits, including any required referrals. We are contractually obligated to collect all patient portions including copays, coinsurance, and deductibles.

Refill requests

Your provider will discuss a treatment plan with you that may include medication. In some cases, refills may be needed prior to a return appointment.

After-hours Emergencies

If you are experiencing a medical emergency, please call 911. However, if you have a post-operative or other urgent medical concern after normal office hours, please call (843) 873-9690 and our answering service can assist with communicating your concerns with the on-call physician.

It is our goal for you to have a comprehensive visit to include any physical examinations, review of records, carefully listening to your questions and concerns and being fully informed of anything discussed during your visit. A visit summary is available to you at the end of each visit upon check out.

Medication Refills

Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. (Visa, Master Card, American Express, Discover.)

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
