How do I sign up for UC Health (MyChart)?

As long as you’ve received care at a UC Health facility, you can still sign up for an account. Click “sign up now.” Complete the sign-up form and submit. My UC Health (MyChart) is a safe and secure online application. All personal information is automatically encrypted and secure.

Who can request access to medical records?

Parents or legal guardians of children and dependent adults may request special proxy access to view portions of their loved one’s medical records. A simple consent form must be completed before access is granted.
