We accept workers’ compensation as payment for covered injuries. Valley Health Care will bill your plan directly for services provided. We abide by all regulations set forth by law.
Does Valley Health Care accept insurance?
Insurance Coverage and Managed Care Plans: Valley Health Care accepts a wide variety of insurance and managed care plans, and will bill your insurance provider directly for the covered services that we provide. Please bring your insurance cards to every visit. Please note that your co-pay for each visit is payable at the time that the service is provided. You are responsible to pay for any services provided that are not covered as part of your insurance coverage. We contract with many plans and continue to negotiate with additional insurance providers.
Does Valley Health Care have sliding fee?
We provide the Sliding Fee Scale payment option for our income-eligible patients based on annual household income. Income guidelines apply. Please be prepared to provide the following acceptable income verification during the application process:
