Foundation Health Partners offers secure electronic Patient Portals at Tanana Valley Clinic (TVC), Fairbanks Memorial Hospital (FMH), and Porter Heart Vascular Center (PHVC). This allows you to manage your health care and access your health information, whenever you need it.
What is the FMH patient portal?
The FMH patient portal is a secure online resource to view upcoming appointments, access your health record, and manage your care at your convenience. You must be at least 18 years old to establish an account and view information. You must be a patient of Fairbanks Memorial Hospital, or have otherwise utilized any of its clinical services.

Information release specialists can provide copies of records for services that occur at Fairbanks Memorial Hospital. Services performed at other facilities need to be requested from those facilities. When was Fairbanks Memorial Hospital built?
Fairbanks Memorial Hospital opened its doors in 1972 thanks to the voluntary efforts and monetary contributions of the citizens of Fairbanks. A 152-bed hospital, FMH serves patients from throughout the Interior of Alaska and provides care in more than 27 specialties.

Medical records is the third door on the left side, sharing an entrance with the Billing office. The Medical Records patient window is to the left side when you enter.
Can a deceased patient be released?
Release of information of a deceased patient may be disclosed based on the presentation of legal documents designating the requestor as the personal representative or administrator of the estate or as provided in the form of written permission by the representative. If there is no personal representative or other person appointed to administer the estate, then records may be released based on following order designated below:
