To activate your account, you will need to receive an activation code from us. Once you have received the code, you will be able to create your own username, password, and other login information used to verify your identity. How to access another patient's portal?

Proxy access allows individuals to view another patient's personal information. Individuals age 18 or older may request proxy access to another individual's Portal account by completing the appropriate proxy access consent form and submitting it to [email protected]. Under state and federal law, there are certain types of medical information that a parent or guardian of a minor child may not view without the minor child's permission. Proxy access for a minor patient will only be granted to parties with parental rights or legal guardianship over the Portal account holder. When a minor patient reaches age 13, the minor patient must also sign a consent in order for a parent or guardian to have proxy access to a minor patient's Portal account. When the child reaches age 18, proxy access is discontinued.
