Welcome to Your Patient Portal! Our hospital knows how important your healthcare is to you – and we value your partnership in maintaining your good health. And, we want to help you keep up with your medical records, appointments and more. That’s why we have created the Patient Portal.
What can I do in the mymemorialconnection patient portal?
By using your MyMemorialConnection patient portal account, you may view, download or request the following: A portion of your medical records and health history List of current medications, allergies, immunizations Request a prescription refill and send to pharmacy of choice
Where can I see my health information from Community Memorial Hospital?
In addition, you will be able to see your health information from Community Memorial Hospital in Ventura, and Ojai Valley Community Hospital. How do I self-enroll for the memorial patient portal?
If you have been a patient at Memorial Hospital or Memorial Physician Clinics after 2014, you can self-enroll for the Memorial patient portal, myMemorialConnection. Just click here to enroll!

What is MemorialChart?
myMemorialChart is a free service offered to patients of Memorial Health System and its physicians.
Where does my memorial chart come from?
Your myMemorialChart information comes directly from your electronic medical record at your provider's office and our hospitals. You can send a myMemorialChart message asking your provider to correct your medical record or call their office directly. You can also have your provider update the information at your next visit.
Can you use my memorial chart for urgent medical matters?
REMINDER: Do not use myMemorialChart for urgent medical matters. If you are experiencing a health-related emergency, call 911 immediately. If you need immediate attention from your physician provider or clinic, call them your physician's office directly.
Does Marietta Memorial Health System have an iPhone?
Marietta Memorial Health System offers Health Records on iPhone, which brings together hospitals, clinics and the existing Apple Health app to make it easy for patients to see their available medical data from multiple providers whenever they choose. Learn more about Health Records on iPhone here.

What is follow my health?
FollowMyHealth is a free health management tool that helps you manage your health care from your computer, tablet or smartphone. You can schedule appointments, view test results, pay your bills, and much more! In addition, you will be able to see your health information from Community Memorial Hospital in Ventura, and Ojai Valley Community Hospital.
Can you enroll in FollowMyHealth if you previously used Community Link?
Patients who previously used Community Link are invited to enroll in FollowMyHealth by creating a new account.
When did we stop uploading patient information to follow my health?
Please note: We stopped uploading patient information to Follow My Health in October 2020. Results uploaded prior to that time will remain in the Follow My Health portal and can continue to be accessed there. Patient information recorded since then will be uploaded to a Cerner portal if one has been created.
Does Apple Health update your health records?
After you add an institution, the Apple Health app automatically updates your health records periodically . If you have any questions, please feel free to contact us via email.
Can you download HeatheLife app on your phone?
You may also download the HeatheLife app onto your phone to access your patient portal.
Can Iredell access my health information?
Iredell is excited to announce that you can now access your health information from your mobile device by downloading and using an approved application.
