How do I reset my INTEGRIS and password?
Please verify your personal information. If you do not remember any of this information, you will have to contact your INTEGRIS and Me help desk at 844-843-1500 to help you regain access to your INTEGRIS and Me account.
How do I add a child to Integris and me?
Go to Health>Share My Record>Friends and Family Access. Click Invite Someone, complete the form, and click Send Invite. If the person to whom you are granting access is not an INTEGRIS patient, or if you are not sure, please complete the Portal Proxy Request for Adult Form and an account will be created for them.

How do I add a child to MyChart app?
For an AdultGo to Your Menu > Share My Record.Under Who Can See My Record?, click Invite Someone.Follow the instructions to send an email invitation to the person who you'd like to have access to your MyChart account. That person will need to accept the invitation and confirm that they know your date of birth.
How do I add another child to my Healow?
To add another child to your app you must click on the settings button in the top right corner and select log out as seen in the screen shot below. This will bring you to the screen below where you can select begin now to add another child.
How do I reset my password?

Can a minor patient be on the patient portal?
In certain circumstances, the patient portal may not show certain medical information about a minor patient. Under state law 63 OK Stat § 63-2602, minor patients have the right to consent for certain services and that information must be kept confidential between the patient and provider.

Room assignments are based upon admitting diagnosis and the bed availability at the time of your admission. Each of our spacious private rooms is equipped with amenities to complement the healing process.
Patient Satisfaction
Because we value your opinion, you will receive a confidential patient satisfaction survey in the mail about one week after your discharge from INTEGRIS Miami Hospital. Please take time to complete the form and return it in the postage paid envelope provided. Your feedback is very important.
