As you find your way along, cancer journey support groups can be excellent resources not only for the information they provide but for the camaraderie of spirit they offer. They can also be helpful at the end of treatment as you find yourself readjusting to everyday life again.

Do you need to complete a new patient form before your first appointment?

If you are a new patient, we ask you to please review and complete our new patient forms before your first appointment. Thoroughness and accuracy are essential when completing your forms, especially your family history questionnaire.

How to contact urgent care?

Suppose I need care but the office is closed. What should I do?#N#If you have an urgent need and the office is closed, you have several options: 1 Call our office at 719-473-2346. The night message provides the contact number for three on-call providers. 2 Visit your nearest urgent care provider. They can assist in most situations. 3 If you are experiencing an emergency, call 911 or go to your nearest emergency room.

Can you discuss your care with another person?

We cannot discuss your care with another person unless we have a signed release-of-information form, which is required by law. Just because you invite a family member to sit in on your appointment does not give us permission to speak with him or her, nor does it serve as a legal release of information.
