Wake Radiology UNC REX provides simple access to your medical and imaging records for services that were performed at any of our outpatient offices. Your results are often needed for a follow-up appointment with another physician. Patients can log into our Patient Portal to access their medical record, results, reports and images.

What time do you call to get your medical records?
Receiving your medical records. For questions regarding your medical record release, please call (919) 784-3158 between the hours of 8:30 a.m. and 4:30 p.m., Monday – Friday. Patients requesting records for their own personal use, fees will apply.
Can you fax a medical record?
For follow-up care, we can fax key medical record information to another care provider upon your request the same day or prior to your scheduled appointment (at no charge). We will not fax for non-patient care purposes.
